Statistics
Lifeworks is currently conducting a statistical survey to determine the number of learning disabled people of all ages in the South West. This information will enable us to identify the funding and services needed by families and individuals who could benefit from respite, life-skills and further education, care and supported living. All information provided will remain confidential, to be used for statistical and research purposes only.
If you know of families or individuals who might take part in this survey, we would very much appreciate it if you could pass our request along, as well as our contact details below. The information we are seeking consists of the following: name, address, birth date, type of disability and type of support required. Just click on the following link to download a pdf form.
